Overview
Figtree House Care Peterborough Ltd is a new care agency in Peterborough who is looking to build a team to lead a rewarding home care company to success. We are looking for a care coordinator who will be working closely with the Registeted manager to provide person centered care from the initial contact to the end of a care package.
The Care Coordinator plays a vital role in ensuring that service users receive comprehensive and effective care. This position involves working closely with healthcare professionals, service users, and their families to facilitate communication and coordinate services. The ideal candidate will possess strong organisational skills and a commitment to providing exceptional support in a healthcare setting.
Duties
* Act as the primary point of contact for service users, families, and healthcare providers, ensuring clear communication throughout the care process.
* Coordinate appointments, follow-up visits, and necessary referrals to specialists.
* Maintain accurate service user records and documentation in compliance with relevant regulations and policies.
* Assist service user in understanding their treatment plans and available resources.
* Collaborate with multidisciplinary teams to develop personalised care plans tailored to individual patient needs.
* Monitor patient progress and address any concerns or barriers to care promptly.
* Provide administrative support within the office environment, including scheduling meetings and managing correspondence.
* Providing 10 hrs of care per week.
* Be part of the on call process
Requirements
* Proven experience in an office setting, preferably within a healthcare environment.
* Level 3 or above in Health and Social Care is essential
* Strong organisational skills with the ability to manage multiple tasks simultaneously.
* Excellent verbal and written communication skills, with a focus on empathy and understanding.
* Ability to work collaboratively within a team while also demonstrating initiative when required.
* Proficiency in using office software and electronic health record systems is advantageous.
* A commitment to maintaining service user confidentiality and adhering to ethical standards in healthcare.
