Overview
Would you be interested in a career within the NHS as a Health Care Assistant in the community? We are looking for a patient centred care provider to support people over the age of 18 living in Bedfordshire.
We are seeking a dynamic, motivated and enthusiastic person with excellent communication, IT literacy and problem solving skills who has a passion for patient centred care. You will be part of a multidisciplinary team (MDT) with clinical and managerial support and aligned to social care professionals as part of the NHS 10 year plan.
Responsibilities
* Deliver quality care to a designated caseload of people in their home, following a care plan devised by community nursing staff.
* Provide feedback in daily handovers to ensure changing needs are met and risks are escalated.
* Be flexible to assist colleagues and support the wider integrated team across locations.
* Maintain patient-centred care and work within the Trust values: We Care, We Respect, We Are Inclusive.
* Maintain documentation and communication in line with clinical and safeguarding requirements.
Location and Team
South Bedfordshire (Dunstable and Leighton Buzzard). Posts available to work with the Dunstable and Leighton Buzzard community nursing teams, delivering care to specified areas and surrounding villages.
Working Pattern and Requirements
* Community Nursing Service operates 08:00 to 21:00 hours with rotational shifts including weekends, evenings and Bank Holidays.
* Access to a car is essential to carry out service duties.
* A full induction programme will be provided.
* Location-specific details: Dunstable, Houghton Regis, Caddington, Toddington, Leighton Buzzard, Linslade, Hockliffe and surrounding villages.
