Registered Nurse – Hospice at Home (22.5 hours per week)
We are a patient centred charity providing palliative and end of life care to thousands of people with advanced life-limiting illnesses Northwest Surrey. We support patients, carers and families at home, in care or nursing homes, or at our In-Patient Unit in the Hospice, and via our Bradbury Wellbeing Centre. This role is vital in the Hospice at Home Team within Community Services, ensuring high quality care in the community.

Clinical Colleague
A successful RN – Hospice at Home will be able to:
1. Support patients with palliative or end of life prognosis and their families / carers to achieve their preferred place of care
2. Work under the supervision of the Clinical Nurse Specialists to develop knowledge and expertise in Palliative care whilst providing a timely service
3. Assist Clinical Nurse Specialists to manage a caseload of patients supporting with telephone calls, home visits, bereavement support and symptom management
4. Provide clinical leadership to junior members of the team with support, teaching and clinical guidance
5. Deliver a high standard of personalised and identify needs of the patient through holistic assessment
6. Use experience and knowledge of palliative care to deliver appropriate symptom control, clinical and personal care, and medication administration
7. Work collaboratively with the Community Specialist Palliative Care Team, GPs and district nursing teams
A successful candidate will be / have:

9. Current NMC Registration
10. RN educated to Diploma or Degree level
11. Palliative care qualification (or willingness to work complete)
12. Mentorship qualification (or willingness to work complete)
13. UK Registration
14. Disclosure and Barring Service Check: The post requires a satisfactory DBS check under the Rehabilitation of Offenders Act (Exceptions Order).
