Overview

Assistant Practitioner Nursing — North East London NHS Foundation Trust (NELFT). This permanent, full-time position (37.5 hours per week) is based at Hawkwell Court and focuses on delivering high-quality nursing care to adults in community settings, including patients’ homes, clinics, and residential care facilities.


Job responsibilities

* Deliver direct patient care, including clinical observations, wound care, and delegated nursing interventions.
* Support the management of long-term conditions and rehabilitation programmes tailored to individual needs.
* Promote patient independence, wellbeing, and self-management through education and support.
* Monitor and report changes in patients’ conditions to the appropriate healthcare professionals.
* Maintain accurate and up-to-date patient records using electronic systems.
* Work collaboratively with nurses, therapists, GPs, and wider multidisciplinary teams.
* Contribute to admission avoidance strategies and facilitate timely hospital discharges.
* Provide compassionate care that upholds patient dignity, choice, and respect.


Key responsibilities and requirements

* Follow an organised approach with a delegated caseload after initial assessment by registered staff, including goal setting, outcomes, risk assessment and discharge planning.
* Adhere to Trust protocols and procedures, particularly relating to administration of medicine and moving and handling.
* Ensure accurate records for audit purposes and effective risk management at team level (accident/incident reporting, risk assessment, and maintaining risk registers).
* Supervise pre-registration students, Band 3 and 2 and unregistered staff as appropriate.
* Contribute to service development with clinicians and Team Lead; participate in team meetings and multidisciplinary information exchange.
* Build relationships to promote seamless care and maintain accurate record keeping.
* Participate in clinical audit and service developments.


Qualifications and essential skills

* Proven ability to deliver direct patient care, including clinical observations and wound care.
* Experience supporting the management of long-term conditions and rehabilitation programmes.
* Competence in promoting patient independence, wellbeing, and self-management.
* Proficiency in monitoring and reporting changes in patients’ conditions and maintaining detailed patient records.
* Strong collaboration skills and experience working within multidisciplinary teams.
* Understanding of strategies to support admission avoidance and timely hospital discharge.
* Commitment to person-centred, compassionate care that respects patient dignity and choice.

Starting and benefits

Starting with NELFT: new starters join the Trust on the first Monday of each month and undergo a comprehensive induction (up to two weeks) including mandatory training, systems training, and equipment allocation, with opportunities to meet executive leadership. Induction location: Rainham, Essex.

High Cost Area Supplement – Outer London: this post attracts an additional 15% of basic salary (minimum £4,714, maximum £5,941 p.a. pro rata for part time).


Probation

This post is subject to a probationary period. Internal applicants are exempt unless already part way through probation or currently a bank member of staff.
