Overview
Reablement and Falls Specialist Physiotherapist. Based at Monnow Vale Health & Social Care Facility, Integrated Service Hub for Central Monmouthshire, working with health and social care colleagues in a community based Integrated Team. Lead on the development and delivery of a specialist falls service for Tier 3 Complex Fallers, engaging the wider MDT, and supporting delivery of a physiotherapy service to Tier 2 Fallers requiring Reablement input. Manage a general community-based caseload as required.
Main duties
* Provide advanced clinical falls risk assessment, clinical reasoning and diagnostic skills for older people with complex falls presentations (unexplained falls, dizziness and giddiness) in community settings. Devise bespoke intervention management and rehabilitation programmes based on specialist assessment findings (may include vestibular assessment and treatment and CBT programmes for service users with fear of falling).
* Independently manage a highly specialist complex caseload, make differential diagnoses and advise on required medical intervention/investigation in a timely manner.
* Provide a wide range of clinical and leadership skills to support the Reablement Physiotherapy caseload and direct management of Tier 2 fallers.
Qualifications
* Essential: Diploma/degree in Physiotherapy; Registered with the Health Professions Council (HCPC)
* Desirable: Postural Stability Instructor qualification (PSI)

Registration: Essential – current UK professional registration with HCPC. UK registration and sponsorship details are as per NHS/UK guidance.
