Main duties of the job
The post-holder will provide high-quality general medical services to the practice population, including the assessment, diagnosis, treatment and management of acute and chronic medical conditions.
* Conducting face-to-face, telephone and remote consultations
* Managing a range of acute and long-term conditions
* Maintaining accurate and contemporaneous clinical records using SystmOne
* Promoting health education and preventative care
* Working collaboratively within the multidisciplinary team

Key responsibilities
Diagnose and manage acute and chronic conditions, integrating drug and non-drug treatment methods into holistic management plans.
Support patients to adopt health-promotion strategies that encourage healthy lifestyles and principles of self-care.
Adhere to evidence-based best practice and national guidelines when treating a wide range of clinical conditions.
Maintain clear, accurate and contemporaneous consultation notes to agreed standards after each patient interaction within the practice clinical system.
Screen patients for disease risk factors and early signs of disease.
Adopt the highest standards of information governance, ensuring patient data is accessed, always shared and stored safely and securely.
Meet with the practice team regularly to provide clinical advice, share learning from local or regional meetings and clinical incidents, and contribute to the clinical strategy of the practice.
Provide advice, support and, where required, clinical supervision to other members of the multidisciplinary practice team.
Attend local or regional clinical meetings on behalf of the practice when required.
Work with the Practice Manager to manage the priorities of key stakeholders, including the Primary Care Network and Integrated Care Board (ICB).
Assist the practice in fulfilling its contractual requirements and achieving key performance metrics, including those specified in QOF, while identifying opportunities to improve quality of care and patient experience.

GMC registration with a licence to practice and inclusion on the GP Performers List.
