Job Description / Person Specification
We are seeking dedicated and compassionate Relief Hub Facilitators to join our North team. This role ensures day opportunities meet care and support needs, working across a geographical area and across multiple buildings.
The role involves providing high‑quality physical and emotional support, respecting privacy, dignity and specific needs, within a risk management framework. Responsibilities include developing care plans, working 1:1 and in groups, using equipment per care plan, reporting health and safety issues, ensuring delivery of day opportunities within policies and legislation, supporting independence, creating care plans and risk assessments, medication administration, safe moving and handling, crisis response, escalation, stakeholder collaboration, and supervising Hub Enablement workers.
Required Qualifications and Experience
* A Social Care qualification or equivalent, with a care qualification or pursuing Level 3 Diploma in Adult Care (supervisory role)
* Knowledge of relevant legislation and ICT systems in the service area
* Evidence of continuous mandatory training and development
* Excellent professionalism, efficiency, and effectiveness in service delivery
* Ability to collaborate well with colleagues and work independently
* Effective communication skills with individuals at all levels
* Experience working with people with disabilities or in care settings
* Experience empowering and advocating on behalf of individuals
* Experience handling money and understanding financial guidelines
* Experience supervising staff and delivering a range of day‑to‑day opportunities
* Minimum of two years’ experience in service delivery in a care setting
