Virtual Ward Qualified Advanced Clinical Practitioner
Lewisham and Greenwich NHS Trust is looking for a qualified Advanced Clinical Practitioner (ACP) to join our new, innovative Virtual Ward service.
Responsibilities
* Deliver autonomous assessment, diagnosis, treatment and management of patients within your scope of practice, ensuring patients can remain safely at home.
* Provide remote and face‑to‑face clinical assessments, focusing on frailty, long‑term conditions and sub‑acute presentations.
* Work collaboratively within an integrated multidisciplinary Virtual Ward team, supporting patients at the interface of community and acute services.
* Document and report clinical findings in accordance with Trust policies and the ACP competency framework.
Qualifications & Requirements
* Qualified ACP with completion of the Lewisham & Greenwich NHS Trust ACP competency framework (required for Band 8a progression).
* Full UK driving licence and reliable access to a car for community visits.
* Ability to work independently and adapt to evolving care models.
Career Progression
While advertised at Band 8a, we will also consider experienced clinicians with potential to develop into the role. A suitable candidate could be appointed at Band 7 in a trainee capacity, with progression to Band 8a upon meeting agreed competencies and portfolio requirements.
