Overview
An exciting opportunity has arisen within our Community District Nursing team for a Senior Band 6 Community Staff Nurse with a Diploma in Integrated Community Nursing or District Nursing qualification.
Responsibilities
* The post holder is responsible and accountable for the delivery of an accessible, appropriate, high quality and effective nursing service to the community by working in partnership with patients, care providers and other agencies.
* Ensure a person-centred approach to care delivery which is safe and effective to support and maintain quality strategy.
* Function as an independent autonomous practitioner by virtue of in-depth knowledge, expertise, proficiency and experience to lead and enable the Community nursing team. Work with the Community Nurse Team Lead to lead/manage and support the Community nursing team; be a clinical expert/senior clinician; be involved in public health activities; be accountable for a caseload; and be a key member of the Integrated Community team and other multi-disciplinary groups.
* Maintain good communication, the ability to work from your own initiative, and as part of a team; evidence of continuing professional development.
* Must be a car driver and hold a full driving licence due to the rural aspect of the caseload.
Qualifications/Requirements
* Diploma in Integrated Community Nursing or District Nursing qualification.
* Current nursing registration and eligible to practice.
* Car driver with a full driving licence.
