We are looking to recruit an enthusiastic Clinical Lead to join Chester South Community Care Team in the Chester area. You will be joining a multidisciplinary team including but not limited to; team manager, enhanced clinical practitioner, clinical leads, community nurses, AHPs, and other specialist professionals.
We seek someone who is enthusiastic, self-motivated, and capable of providing confident support, leadership, and coaching to clients and clinicians. Excellent communication skills and the ability to work in partnership with clients and their families/carers are essential. The role also involves safeguarding individuals at risk from harm and abuse.
Key Responsibilities:
1. Provide a visible clinical profile with leadership and oversight of a designated Care Community caseload.
2. Collaborate with wider health, social care, and third sector partner agencies to meet patient needs.
3. Ensure patients receive appropriate care from the right person, at the right time and place.
4. Center all decisions around the patient, promoting a streamlined patient journey with minimal delays and a focus on independence and quality of care.
5. Provide direction within the caseload considering current health and social care agendas and local initiatives.
6. Be aware of local public health community profiles and their influence on care requirements.
7. Understand the skills needed from the Community Care Team based on community profiles.
8. Focus on delivering person-centered care.
9. Manage and monitor care standards to provide effective, evidence-based care to a defined population.
10. Provide clinical leadership, coaching, and development for the clinical team.
