Overview
This is a Band 6 Community Learning Disability Nurse position within the Wrexham Learning Disability Community Health team at Betsi Cadwaladr University Health Board (BCUHB). The post holder will work as part of the Learning Disability team based in Wrexham, ensuring interfaces between the Local Authority and neighbouring services within the Division including community mental health teams, in-patient services, and other statutory agencies and the voluntary sector. The post holder will deliver evidence-based, safe, effective interventions and clinical care, following the nursing process. The ability to speak Welsh is desirable; Welsh and/or English speakers are equally welcome to apply.
Responsibilities
* Deliver safe, effective and high-quality specialist services in line with agreed standards and service principles for people with a learning disability.
* Act as a visible and accessible member of the team; provide support, advice and information to patients and their families.
* Communicate and interpret highly complex and sensitive information and act appropriately where there may be barriers to acceptance.
* Understand the CHC process and contribute to the development of pathways and the Maturity Matrix.
* Collaborate with the Local Authority, other services, and statutory/voluntary sector partners to coordinate care.
Qualifications and Essential Criteria
* RNLD Qualification
* Evidence of a degree-level qualification or equivalent in line with NMC registration
* Evidence of postgraduate education and ongoing CPD
Desirable Criteria
* Management qualification and/or management experience
* Mentor/supervisor/assessor experience
Experience
* Significant experience within the clinical speciality or a relevant nursing role (essential).
* Experience of working in a multidisciplinary team setting (desirable).
* Experience/knowledge of senior nurse role (desirable).
Aptitude and Abilities
* Ability to communicate and receive complex information
* Ability to diagnose a problem and take corrective action
* Ability to analyse assessment results and translate them into relevant packages of care
* Ability to work autonomously as well as part of a team
* Desirable: Welsh language ability
