Overview
The support worker will be part of an exciting new Intensive Home Treatment Team (IHTT) with Buckinghamshire CAMHS in concordance with NHSE guidance. The team will support individuals with complex needs requiring intensive risk support within the community. The post-holder will be part of a multi-disciplinary team that develops formulations to provide holistic care to young people and families. The post holder will work alongside other clinicians in holding a shared caseload of young people who require active support and/or treatment, with the aim of delivering individualized home and community treatment packages.
Responsibilities
* Deliver clinical care and manage a caseload, including developing and evaluating care plans with qualified staff and supporting individual recovery needs.
* Maintain accurate and timely clinical records, ensure confidentiality, and contribute to risk assessments and safeguarding procedures.
* Support service users in emergency situations, provide handovers, and implement recommended strategies across professional networks.
* Attend and contribute to meetings, including clinical reviews, care planning, and service development discussions.
* Liaise with leadership and undertake additional duties as required to support service provision and team operations.
* Support service users across Buckinghamshire in a variety of settings (school, local community, home, clinic).
* Support IHT staff with the assessment and treatment of young people as required.
* Manage a small caseload in conjunction with qualified clinicians, with at least 3 contacts per week up to multiple contacts per day.
* Flexible working across the week to support IHT provision for young people and families.
Requirements
* All new starters have a thorough induction process, locally and Trust wide, to learn key elements of the role over a reasonable timescale.
* Appointment to this post is subject to satisfactory references covering 3 years of employment or study; referees should respond promptly to requests.
