About the role
The Care Coordinator will be responsible for:
• Conducting needs assessments and developing individualised care plans for service users.
• Ensuring continuity and excellence of service delivery through effective management, planning, and implementation of a robust rota system.
• Supervising care staff and teams, providing guidance, support, and mentoring, including demonstrating procedures and coaching as required.
• Being available to care staff for advice and guidance on operational or care-related issues.
• Providing direct care during emergencies or out-of-hours situations as required.
• Conducting spot checks on care delivery and reporting findings to service management.
• Visiting service users to obtain feedback on the service provided and addressing any concerns appropriately.
• Undertaking preliminary investigations into concerns, complaints, or safeguarding issues arising during service delivery, and reporting findings to the service manager.
• Organising care reviews and ensuring any changes to care plans are implemented effectively.
• Reviewing daily notes, journals, and records of incidents or concerns.
• Providing support to the senior management team, including Registered Managers (RMs).
• Maintaining up-to-date knowledge of company policies, procedures, and regulatory requirements.
• Liaising with other professionals involved in the care of service users to ensure coordinated support.
• Collaborating with the management team to support quality improvement initiatives and business planning strategies.
• Engaging in continuing professional development, including specialist training and relevant qualifications.
• Working as an effective team member and demonstrating flexibility to meet service requirements.
• Ensuring compliance with the Health and Care Act 2012 and the Care Quality Commission (CQC) 5 Domains: Safe, Caring, Effective, Responsive, and Well-led.
• Adhering to national guidance, including NICE guidelines, Skills for Care standards, and relevant professional forums and support networks.
• Undertaking any other administrative or ad-hoc duties as reasonably required by the manager.
Desirable
• Level 3 Diploma in Health & Social Care (or working towards it)
