When you join the Visiting Nurse andHospice for VT & NH you ll become part of a dedicated teamdelivering outstanding home health and hospice services that enrichthe lives of the people we serve. RN Case Managers at the VNHscreen evaluate reevaluate and treat patients as well as coordinateand communicate treatment activities with our interdisciplinaryteam members and the patient. The RN Case Manager:Develops a careplan in collaboration with the patients physician the patient andtheir family or the primary caregivers upon admission. Periodicallyreviews the patient care plan and recertifies the patient care planas appropriate. Coordinate the patients care plan throughinterdisciplinary communication and case conferences asappropriate. Advise other disciplines of discharges in a timelymanner.Provide ongoing assessment of the patients clinicalfunctional and emotional status to identify problems and/orprogress toward desired goals. Communicate significant changes tothe patients physician and VNA colleagues.Document evaluationstreatment goals and objectives.Teach the patient and caregiverselfcare techniques as appropriate. Provide medication diet andother instructions as ordered by the physician and utilizesopportunities for health counseling with patients and caregivers.Minimum Qualifications:Must be a licensed Registered Nurse inVermont and New Hampshire or hold a compact license.Graduate of anaccredited school of nursing.Medical/Surgical nursing and/or homehealth care experience.
