Job Summary
We are seeking a dedicated and organised Care Coordinator to join our team. The ideal candidate will play a crucial role in ensuring that our clients receive the highest standard of care and support. This position involves coordinating care services, liaising with healthcare professionals, and maintaining accurate records to facilitate effective communication among all parties involved in the care process.
Duties
* Coordinate and manage care services for clients, ensuring their needs are met efficiently and effectively.
* Liaise with healthcare providers, families, and clients to facilitate communication and ensure continuity of care.
* Maintain accurate and up-to-date records of client information, care plans, and service delivery.
* Assist in scheduling appointments and managing client calendars to optimise care delivery.
* Provide administrative support within the office, including handling correspondence and managing documentation.
* Monitor client progress and report any changes or concerns to the appropriate healthcare professionals.
* Participate in team meetings to discuss client care strategies and improve service delivery.
Experience
* Previous office experience is essential for this role, demonstrating strong organisational skills and attention to detail.
* Experience in a healthcare or social services setting is advantageous but not mandatory.
* Proficiency in using office software applications for record keeping and communication is required.
* Excellent interpersonal skills with the ability to communicate effectively with clients, families, and healthcare professionals.
