Job Purpose

The Neighbourhood Health Engagement Lead will play a key role in strengthening partnerships between HWW Primary Care Network, patients, and the Voluntary, Community, Faith and Social Enterprise (VCFSE) sector. The postholder will be responsible for mapping local assets and organisations, fostering collaboration, and supporting meaningful patient engagement. They will develop, deliver, and evaluate collaborative health and wellbeing initiatives, ensuring that the voice of local people is central to neighbourhood-level planning and delivery.

Key ResponsibilitiesCommunity Mapping & Asset Development

· Lead on mapping local VCFSE organisations, groups, and community assets.

· Maintain an up-to-date directory of local resources and services to support PCN teams and patients. Consider use of digital tools such as Wirral Infobank.

· Identify gaps, opportunities, and strengths in community provision.

Patient & Public Engagement

· Work with Patient Participation Groups (PPGs) that are within HWW practices to support effective engagement and ensure diverse voices are heard.

· Facilitate patient and carer involvement in service design and evaluation.

· Organise listening events, focus groups, and other neighbourhood events.

· Utilise social media to support engagement.

· Attend community events and networks.

Partnership & Relationship Building

· Develop and maintain strong, trusted relationships with VCFSE partners, community leaders, and other stakeholders.

· Act as a key link between PCN clinical and non-clinical teams and the wider community sector.

· Support information-sharing and partnership working across health, care, and community sectors.

Collaborative Projects & Social Action

· Co-design and deliver small-scale collaborative projects with VCFSE organisations to address local health priorities.

· Facilitate social action initiatives that promote health equity, resilience, and wellbeing.

· Monitor, evaluate, and share learning from collaborative projects.

Strategic Contribution

· Provide insights and intelligence from community engagement to inform PCN priorities and planning.

· Contribute to reducing health inequalities and improving population health outcomes supported by the wider One Wirral team.

· Support integration between health, care, and community services at neighbourhood level supported by the wider One Wirral team.

Person SpecificationEssential Skills & Experience

o Experience of working in or alongside the VCFSE sector, with strong understanding of community development and partnership working.

o Experience of engaging patients, carers, and communities in service design or improvement.

o Strong relationship-building, networking, and stakeholder management skills.

o Proven ability to design, deliver, and evaluate community-led projects.

o Excellent communication and facilitation skills, able to engage diverse audiences.

o Knowledge of the health inequalities agenda and the role of PCNs in addressing it.

o Strong organisational and project management skills, with ability to work independently and collaboratively.

Desirable Skills & Experience

o Experience of working in a primary care, health, or social care setting.

o Understanding of co-production approaches.

o Knowledge of local health and care systems and structures.

o Experience of supporting or facilitating Patient Participation Groups (PPGs).

Other Information

o This is a part-time post (3 days / 0.6 FTE) with a salary of £33,000 pro rata per annum (actual salary £19,800 per annum).

Pay: £19,800.00 per year

Expected hours: 22.5 per week
