Our Healthy Child Programme 0-19 (25 SEND) Service for Health Visiting and School Nursing aims to improve health outcomes and influence wider determinants of health through collaboration with the community and health and care partners. We are currently undergoing an exciting service transformation in partnership with our team.

You will provide compassionate, community-based support to children, young people, and families in Tameside, helping them stay healthy and reducing unnecessary hospital visits. You will manage a caseload of children and deliver the Healthy Child Programme, promoting the best start in life and school readiness.
You will lead and manage the delivery of the Healthy Child Programme, work with complex cases including safeguarding, and identify children with Special Educational Needs and Disabilities early. You will signpost and support children, young people, and families, collaborating with neighbourhoods, communities, voluntary, and statutory agencies. We deliver the Greater Manchester Early Years Delivery Model and are committed to improving school readiness rates.
