The Role
An exciting opportunity has arisen for an experienced registered nurse who already holds a first level adult nursing degree to join our OOH evening service as a community band 5 staff nurse.
Applicants must have significant nursing experience, demonstrable leadership and communication skills, evidence of continuing development, be passionate about a career in community services and be motivated to work autonomously within a dynamic environment as part of the wider HSCP. As a community staff nurse you will work within the Primary Health Care team and under the direction and supervision of the District Nurse. You will assess and deliver an agreed plan of care to individuals within a community setting. A clean current driving licence and access to own car is required.
Duration, Location, and Working Pattern
* Permanent, part‑time position, 12 hours per week.
* Location: Milend Centre, Seedhill Road, PA1 1SA.
* Shift pattern: 3 evenings (1730‑2130) over 7 days including weekends and public holidays.
* Salary is pro rata for part‑time hours.
Key Responsibilities
The post holder is responsible and accountable for assessing, planning and implementing programmes of care as directed by the district nurse. These will be developed in partnership with patients, family, carers and significant others, and will be evidence based taking into account lifestyle, race, gender and cultural background, and factors impacting on physical, mental or emotional health.
The post holder may also be required to assume the caseload holder role during short periods of sickness, absence and annual leave, seeking support as required from other senior members of the district nursing team.
* Utilise various clinical and environmental risk assessment tools to identify actual and potential risks and ensure implementation of appropriate interventions.
* Report and participate in the critical appraisal of incidents to reduce any recurrences and participate in further investigation as required.
* Support patients, families and carers in order to achieve optimum health and independence.
* Plan and prioritise workload, delegate work using available resources and make decisions to ensure that patients’ needs are being met.
* Work within systems to ensure continuity of a quality 24‑hour nursing service.
* Provide care of patients within a Treatment Room or Health Centre setting when required.
* Use a systematic, flexible and innovative approach to problem‑solving when there are barriers to understanding or financial constraints within the patient’s home, in order to provide a safe and effective care package.
* Participate in clinical audit and research to ensure continuing improvement in practice and enhance individual and team performance for the improvement of patient care.
Qualifications & Experience
* Part 1 NMC Registration.
* or 2nd Level Registration with general nursing experience.
* Evidence of continuing professional development.
* Evidence of revalidation.
* IT skills – word processing, email, Internet.
* Good communication skills (both verbal and written).
* Ability to work on own initiative.
* Leadership skills.
* Problem‑solving skills.
* Ability to work as part of a team.
* Full driving licence.
