We are excited to offer a new role for a self-motivated TherapyAssistant to join our therapy services and help make every moment count.
As a Therapy Assistant Practitioner, you will play a vital role insupporting the delivery of high-quality therapy services within an inpatientand community setting.
Working closely with our Therapeutic and Nursing teams, you will helppatients maximise their independence, dignity and comfort.
Main duties of the job
What You'll Do:
* Support thedelivery of therapeutic and rehabilitative care, managing a delegated caseload
* Undertake patientassessments, implementing and evaluating therapeutic interventions, andsupporting discharge planning.
* Maintainaccurate documentation and contribute to service development.
* Provide emotionalsupport to patients and their families with compassion and sensitivity.
What Youll Need
* NVQ level 3 or equivalent
* Relevant experience of working as a Therapy Assistant orClinical Support Worker
* Have excellent communication and interpersonal skills
* Experience of managing own caseload
* Show empathy, compassion, and a commitment to highstandards of care

Job responsibilities
* Develop therapeutic partnerships with patients tomaximise their independence in context of their potential.
* Undertake assessment and intervention autonomously,both on an individual and group basis as appropriate, within a designatedcaseload and within scope of competence.
* Gather patient information as required, usingavailable resources, including interviews with patient and/or carer, clinicalrecords, liaison with other staff involved in the patient's care, internaland external agencies.
* Coordinate and lead therapeutic activities in the WellbeingCentre and In-Patient Unit, working with and delegating to other supportstaff, including volunteers, within scope of role.
* Negotiate and agree functional treatment goals withthe patient using a range of technical and creative skills as delegated bytherapy staff.
* Deliver therapy treatment plans as delegated anddesigned by the Occupational Therapist and/or Physiotherapist. These planswill have a rehabilitative or compensatory approach as needed to enablepatients to participate in treatment episodes and carry out activities ofdaily living.
* Provide general advice to patients, relatives andcarers to enable understanding of the aims of the prescribed intervention,ensuring there is a consistent approach to patient care and offeringinformation on hospice and community services.
* Evaluate patient progress, reassess and altertreatment programmes if required, in discussion with the Therapy team wherenecessary.
* Be responsible for the agreed discharge of patientsfollowing completion of an episode of care under the guidance of clinicians.
* Visit patients in their own homes to assess forsuitability of equipment required for discharge, and identify when furtherreferral is required as delegated by the OT.
* Engage in clinical and management supervision.
Administration and Communication
* Be a member of an integrated team, sharinginformation with team members and having an awareness of operatingprocedures.
* Participate in and/or facilitate multidisciplinaryprofessional meetings representing both the MDT as required, e.g. staffforum.
* Communicate effectively with patients with complexcommunication difficulties and/or cognitive problems by utilising a varietyof communication media.
* Responsible for maintaining accurate clinicalrecords for a designated caseload.
* Be responsible for collecting clinical statistics,as delegated by MDT.
* Participate fully in service development asrequested by therapy staff.
* Participate in project working groups/audit, asrequested by therapy staff.
* Liaise with, coordinate and facilitatemulti-disciplinary and multi-agency referrals for patients and carers.
Person Specification
Experience
* Relevant experience of working as Health Care Assistant/ Physiotherapy assistant/Occupational Therapy assistant.
* Experience of running groups.
* Competent IT skills, including email and the ability to navigate and use a clinical IT system.
* Experience of managing own caseload.
* Experience of working in palliative care.
* Experience of working with Volunteers.
Qualifications
* NVQ level 3 or equivalent.
* Care Certificate, or working towards.
