Caseload Manager Band 6

An exciting opportunity has arisen for motivated individual to work within the Community Nursing Division as a Community Nursing Caseload Manager, within the Wallasey Locality working with the Pasture Road Community Nursing Team in Moreton.

Main duties of the job

This is a permanent contract .

.5 hours per week. Hours to be discussed at interview.

This post will be responsible for managing planned programmes of care for patients in domiciliary and clinic settings.

A main part of the role of a Caseload Manager is to assess, plan, implement and evaluate nursing care to patients across the community/primary care interface in the most efficient and clinically effective way, in co-operation with other members of the health and social care teams, and voluntary and statutory agencies.

Our Community Nursing Team supports the needs of housebound patients living on the Wirral. We provide valuable care, supporting people to manage their long term conditions, and maximise their independence, which in turn can prevent admission into hospital or enable a quicker discharge if they admitted.

You will need to be able to work autonomously and as part of a team and possess excellent time management, organisational and communication skills. You will be innovative, flexible and prepared to adapt to the changing needs of the Trust as they arise.

Person specification

Qualifications and professional training

Essential criteria

1. Registered General Nurse educated to degree level (or equivalent)
2. Current NMC registration
3. Specialist Practitioner Qualification – District Nursing (or equivalent postgraduate modules) or equivalent level nursing qualification/ experience
4. Evidence of further recent study/professional development

Desirable criteria

5. Independent non-medical prescriber or willing to undertake
6. Teaching/Mentorship Qualification
7. Clinical diagnostics module (or willing to undertake)
8. Clinical examination module (or willing to undertake
9. Leadership Qualification – ILM level 6 or equivalent (or willing to work towards) or relevant experience

Experience and Knowledge

Essential criteria

10. Experience of managing a caseload/leadership within the community
11. Knowledge and experience of managing patients with long term conditions
12. Knowledge of tissue viability, including pressure and leg ulcer management
13. Knowledge and experience of end of life care
14. Knowledge and experience community nursing conditions
15. Awareness of Workforce Policies and Procedures
16. Evidence/experience of teaching/mentoring
17. Experience of Multi-disciplinary working
18. Supervisory and/or undertaking clinical supervision experience
19. Awareness of professional issues and developments
20. Understanding of national government initiatives in health and social care and how they impact on local services
21. Experience of clinical assessment, analysis, planning, delivery and evaluation skills
22. Ability to manage complex cases
