Leading, influencing, and delivering a comprehensive Health Visiting service in line with the Healthy Child Programme and local service specifications, addressing the public health needs of local communities.
You will undertake the Health Visitor role for 80% of your clinical time. 20% of your role will be dedicated to working within a structured 6-9 month work-based programme under the supervision of the Public Health Nurse Consultant. You will identify, design, and manage a project focusing on health at a population level, aiming to make communities and environments healthier, reduce ill-health, and tackle health inequalities.
* Focus on health at a population level, enhancing community and environmental health, reducing ill-health, and addressing health inequalities.
* Measure, monitor, and report on population health and wellbeing, including health needs, risks, and inequalities for a specific area.
* Audit, evaluate, and redesign services and interventions within your area to improve health outcomes and reduce inequalities.
* Work with policies and strategies aimed at improving health outcomes and reducing health inequalities.
