Health Care Support Worker - Southern Parishes ICT

To work within the multi-disciplinary Integrated Care Team that delivers community based services to a GP Practice population, undertaking nursing tasks within agreed parameters, set out by the registered practitioners.

The job will include home visiting and lone working without direct supervision.

The scope of the role will focus on supporting admission avoidance, early supported discharge and promotion of independence and assisting in the effective management of patients with long term conditions and older people with mental health problems.


Main duties of the job

To provide a supporting role to registered practitioners in the Integrated Community Team in the delivery of nursing care respecting patients' privacy and dignity at all times.

To act as key worker for patients under the direction of the registered practitioners.

To assist in planning, delivering and evaluating programmes of care prescribed by the registered practitioners, regularly liaising at the beginning and throughout each shift.

To observe, monitor and record patient activity and health status as appropriate, communicating any changes in their condition to the registered practitioners.

To be responsible for progressing patient's treatment plans within guidelines set by registered practitioners and regularly contributing to the review of each patient's progress within the team.

To perform a range of clinical nursing skills as directed by the registered practitioners:

Vital Signs - Temp/Pulse/Lying and Standing BP/Respiratory Rate

Mouth care/Eye care/Toe nails and Finger nails

Catheter care/Bladder washouts/Stoma care

Continence re-assessment

Venepuncture

Basic Wound care

Simple bandaging and paste bandaging

Short stretch bandaging under supervision and after training

To provide information and advice to patients and carers on how to optimise functional ability, nutrition, health and safety, and health promotion.

Person Specification


Qualifications

* Literate
* Numerate
* Relevant experience as a care assistant in a health care environment.
* Evidence of delivering care delegated from a range of registered professionals, in particular nursing.
* Evidence of ability to implement prescribed programmes of care for a range of conditions.
* NVQ 3 in health care related subject or willingness to undertake
* FULL UK DRIVING LICENCE is required at time of applying for role.
* (GOV website states - Driving in Great Britain on a non-GB licence
* You can drive in Great Britain on your full, valid driving licence for 12 months from when you became resident
* After 12 months you'll need to apply for a provisional licence and pass the theory and practical driving tests to drive in Great Britain.)
* As this role is out in the community, we cannot accept applicants with a non-GB licence
