Role details
This role is offered on a permanent basis. The hours are part-time, 30 hours per week. The Stroud Assertive Outreach Team provides comprehensive mental health and social care to working age adults.
Responsibilities
The post will include care coordinating a designated caseload, working within CPA Policy and associated countywide risk procedures, travelling independently, use of IT skills and a working knowledge of legislation regarding safeguarding vulnerable adults, the Mental Health Act (MHA) and Mental Capacity Act (MCA). The specific purpose is to deliver care in a collaborative manner that reflects the principles of Assertive Outreach Team (AOT) working, particularly aspects relating to recovery and social inclusion.
We are looking for an enthusiastic and motivated individual to join our multi-disciplinary team. Applicants should have an interest in working with individuals who have severe and enduring mental health problems and are difficult to engage.

The Stroud Assertive Outreach Team provides comprehensive mental health and social care to working age adults. We have a skilled and dedicated workforce of over 5000 colleagues across 55 sites and in people's homes.
