We are looking for an experienced Team Coordinator to support a team of support workers and a young adult with complex neurological and mental health needs in the Northampton area. You will need to be available immediately. As a Team Coordinator, you will play a key role in both d elivering care and overseeing the day-to-day running of the package, ensuring consistency, quality, and a structured approach to support. You will be responsible for line managing Support Workers within the team, providing guidance, support and informal training, as well as undertaking regular 1-1s and contributing to performance and development. You will need to have a proven track record of working within complex care and supporting individuals with: Acquired Brain Injury (ABI) Mental health needs, including emotional regulation and behavioural support Cognitive and communication challenges Supporting independence, routine and community engagement Risk awareness, including vulnerability within the community You will be expected to: Lead by example and deliver high-quality, person-centred care Support the client with daily routines, activities and engagement in the community Provide clear, calm and consistent communication Work closely with family and the wider multidisciplinary team Monitor and report on changes in presentation, behaviour or risk Ensure consistency across the team and maintain a structured approach to care You must hold a UK driving licence and have access to a car.
