Job Summary

We are seeking a highly motivated doctor to join our dynamic Acute Frailty Service as an Acute Frailty Fellow. This post is ideal for doctors with at least 12 months' experience at FY2 level or above, including some exposure to Geriatric Medicine, who wish to develop specialist skills before committing to a career pathway in Elderly Care .

The successful candidate will work within A&E and Acute Medicine at the Queen Elizabeth Hospital, providing high-quality assessment and management of frail older adults aged 65 and over. You will be part of a well-established multidisciplinary team delivering Comprehensive Geriatric Assessment and supporting safe, timely discharge.

Weekend working (08:00-16:00, Saturday/Sunday) forms part of the rota and attracts pay enhancements.

Main duties of the job

Clerking, assessing and managing frail older patients in A\&E
Developing individualised care plans with a focus on geriatric syndromes
Conducting advanced care planning, DNAR and end-of-life discussions with patients and families
Participating in daily MDT meetings
Liaising closely with discharge coordinators, community teams, GPs and care homes to facilitate safe discharge
Making referrals to other services as required
Contributing to audit, quality improvement and service development
Participating in teaching for junior doctors, medical students, nurses and AHPs
Engaging in departmental educational activities and postgraduate training
Supporting Trust-wide clinical governance, M\&M and Safecare processes

This post would suit an experienced resident doctor in medicine with an interest in Care of the Elderly and an aspiration to further their experience in the speciality before committing to a future career path.

Responsible for the clerking and day to day care of patients identified by the Frailty Team in A&E. They will undertake independent clinical review of patients and develop individualised care plans with particular reference to geriatric syndromes.

Undertake sensitive discussions with patients, families and carers around advanced care planning, DNAR discussions, admission avoidance planning, NG feeding/risk feeding, end of life discussions and explaining clinical management plans to patients and their families.

Make appropriate referrals to other health and social care professionals.

Work proactively and collaboratively to expedite discharges, working closely with social care teams including discharge coordinators, discharge hub teams, community teams, GPs and care homes.

Participate in daily Multi-disciplinary Team meetings and work closely with Allied Health professionals.

Participate in audits, quality improvement projects and Frailty service development.

Contribute to the delivery and development of education and training pertaining to the Frailty work-stream.

Attend the active postgraduate programme with weekly lectures, radiology, medical and pathology meetings and medical unit presentations.
