Job Description and Responsibilities
Registered Nurse — 30 hours a week (permanent post)
£19.69 an hour, annual salary £30,716.40
Are you a kind, caring and compassionate staff nurse with a passion for providing high quality palliative and end of life care? If so, come and join our team working across our In-Patient Unit and Hospice at Home services. Education and full preceptorship program, supporting your development is provided.
Key responsibilities
* Undertake holistic individualised patient assessments using evidence-based tools to assess physical, psychological, social and spiritual needs.
* Assess the needs of family members and liaise with Family Support Services.
* Integrate OACC and other outcome measures within assessments.
* Deliver evidence-based palliative and supportive nursing care with the patient at the centre of care decisions and their family as appropriate.
* Review patient care with nursing and multi-professional teams at regular intervals, including daily review meetings and weekly multi-disciplinary team meetings.
* Review, evaluate and update nursing care plans and progress reports; maintain accurate documentation and reporting on EMIS and care plans.
* Be accountable for safe custody and administration of medicines and maintenance of records; maintain up-to-date knowledge of drugs’ purpose, actions and side effects.
* Assess and report on the effects of medications and other interventions on symptom management.
* Undertake non-pharmacological interventions to support symptom management (e.g., breathing and relaxation techniques).
* Ensure patient/family confidentiality at all times; comply with policies, procedures and current legislation.
