As a band 6 district nurse, you will be supporting the band 7 in managing the caseload of patients with complex and diverse healthcare needs. The role involves holistic patient assessment, care planning, and delivery, ensuring high standards of care while promoting patient independence.
As a Band 6 District Nursing Deputy Team Lead you will be responsible for providing highly skilled nursing care to patients who reside in the Borough of Newham. You will be promoting the health of this population and reducing the effects of ill health, disability and maximizing the individuals potential for independence.
The Community health Newham extended primary care team is an established integrated service. This will give you the opportunity to work closely with our Physiotherapists, Occupational Therapists, Care Navigators and clinical psychologist. Our service is also supported by a team of experienced administrators.
Main duties of the job
1. The post holder will support the team in:
2. Delivering safe, effective, and evidence-based nursing care to patients in their own homes or community clinics.
3. Ensuring that all patients have a personal care plan developed jointly with the patient, carers, relatives and health & social care professionals, based on a full assessment of care needs.
4. Case managing patients discharge from hospital, by working in partnership with hospital ward staff, social services and other appropriate agencies, including the voluntary sector
5. Working with the multi-disciplinary team promoting self-care for patients with long term conditions.
6. Working with patients and their families to plan for and improve last years of life care, ensuring that choices are reflected in personalized care plans and communicate with others involved in their care.
7. Ensure patients needing palliative care; receive a high standard of care, aligned to the Gold Standards Framework
8. Specialist palliative care services including St. Joseph’s Hospice in developing appropriate End of Life Care Pathways

Essential criteria
9. •Registered Nurse with evidence of current NMC registration
10. •Specialist Practitioner (District Nursing) Degree or equivalent/ Certificate/diploma
11. Community prescriber
Desirable criteria
12. •Leadership or management Course
13. •Physical assessment / First Contact Course (willingness to undertake)

Experience

15. •Experience of working independently, unsupervised providing high quality, evidenced based care.
16. •Able to demonstrate ability to communicate effectively at all levels.
