Your new role
The post holder will be responsible for delivering tailored interventions to identified service users, with the aim of engaging individuals in the most appropriate services for their needs. This approach is designed to reduce re-presentations and support positive, sustainable outcomes. A key aspect of the role involves strengthening joint working protocols between mental health and substance misuse services, including collaboration with external partners for full access to all services.
What you'll need to succeed
The ideal candidate will have experience working within substance misuse and/or mental health settings, with a particular focus on supporting individuals with co-occurring conditions. They will demonstrate a strong understanding of safeguarding best practices and have a proven track record of managing a caseload of clients with complex needs.
