Role
We are seeking a skilled nurse to join our Rapid Response Team in Maidstone as a Team Leader. Your responsibilities will include:
* Conducting comprehensive assessments, planning, implementing, and evaluating holistic patient care.
* Developing personalized care plans in collaboration with patients.
* Coordinating equipment orders and facilitating referrals.
* Serving as a senior practitioner and providing guidance to junior staff on assessments and pathways for patients with complex needs.
* Contributing to the multi-professional team to deliver high-quality care.
* Performing advanced clinical interventions and promptly addressing patient deteriorations.
* Managing a caseload of patients with complex needs.
* Supporting duty roles, including triage.
* Assisting the Clinical Lead in team management.
* Providing strategic recommendations to senior management for service improvements and compliance.
Additional working pattern: Must be available for Early, Late, Night shifts, and Weekends (1 in 4).
