Offer description

Are you interested in joining a Trust committed to your development?

Would you like to be part of an organisation that really values your well-being?

Do you want to treat people rather than conditions?

Here at KCHFT, we are constantly working to improve the lives of our staff and patients. We care for you so you can care for your patients.

Our “We Care” strategy aims to put our communities first, deliver a better patient experience, and be a great place to work for all our staff. These aren’t just words. Our “Outstanding” rated trust has more than half of our workforce working flexibly, including compressed hours and term-time only. Over 70% of our staff feel we take positive action on health and wellbeing, and we strive to improve this. We consistently score above the national average in staff survey results, especially regarding workforce well-being.

With the NHS vision focusing on community care, now is an excellent time to join us.

Role

We are seeking a skilled nurse to join our Rapid Response Team in Maidstone as a Team Leader. Your responsibilities will include:

* Conducting comprehensive assessments, planning, implementing, and evaluating holistic patient care.
* Developing personalized care plans in collaboration with patients.
* Coordinating equipment orders and facilitating referrals.
* Serving as a senior practitioner and providing guidance to junior staff on assessments and pathways for patients with complex needs.
* Contributing to the multi-professional team to deliver high-quality care.
* Performing advanced clinical interventions and promptly addressing patient deteriorations.
* Managing a caseload of patients with complex needs.
* Supporting duty roles, including triage.
* Assisting the Clinical Lead in team management.
* Providing strategic recommendations to senior management for service improvements and compliance.

Additional working pattern: Must be available for Early, Late, Night shifts, and Weekends (1 in 4).

Rated ‘outstanding’ by the CQC, we are passionate about high standards of patient care and community health. We seek outstanding individuals who share our values: compassionate, aspirational, responsive, and excellent.

We value equity, diversity, and inclusion, believing they lead to better patient care and a happier workforce. We welcome applications from diverse backgrounds and collect data to promote equitable opportunities.

As a flexible organization, we support flexible working arrangements that suit both staff and service needs. Discuss your preferences with us.
