Overview

Advanced Nurse Practitioner (Independent Prescriber) – Birmingham / West Midlands, United Kingdom. The ANP will work providing care for presenting patients from initial history taking and clinical assessment through to the diagnosis, treatment and evaluation of care. They will demonstrate critical thinking in the clinical decision-making process, including assessment and diagnostic skills, leading to the delivery of safe care for all patients. They will work collaboratively with the healthcare team to meet the needs of the patients, supporting the delivery of policy and procedures. The post holder will act as an autonomous practitioner working independently in Taurus Healthcare Clinical service areas to assess, diagnose and treat the conditions of patients attending within primary care services and their own homes, initiating direct referrals as appropriate.

Responsibilities include leading prescribing as the lead clinician, supporting nursing services, and taking overall clinical responsibility for clinical areas where a GP is not physically present (GP support available if needed, which may be virtual). The post holder must possess competencies across all four pillars of advanced practice.


Key Responsibilities

* Undertake safe clinical decision-making and expert care, including assessment and diagnostic skills and safety netting for patients using face-to-face (clinic and home visits), telephony and online consultation platforms.
* Demonstrate critical thinking in clinical decision-making, prioritise and triage patient needs, and initiate appropriate investigations or referrals to colleagues and other care providers.
* Follow local and national guidelines and pathways, work to the latest evidence base, and practise reflectively, completing significant event analyses and audits to evaluate care effectiveness.
* Support and manage health needs of women presenting for family planning, cervical cytology or sexual health consultation.
* Use advanced clinical skills to educate service users, promote self-care and empower informed treatment choices.
* Prevent inappropriate hospital admissions by coordinating specialist care in the most appropriate environment; work collaboratively with hospital and community colleagues to ensure timely and appropriate care.
* Refer patients to other medical specialities and to multidisciplinary teams, secondary care, and other statutory/voluntary organisations; include referrals for imaging (e.g., x-ray).
* Undertake and interpret a range of diagnostic tests and routine clinical procedures; work independently in home environments as required.
* Direct, provide and evaluate the highest standard of evidence-based clinical care to patients in their own homes, based on agreed management care plans and protocols.
* Prioritise own clinical workload and collaborate with the wider clinical team to manage day-to-day demand and priorities; intervene in complex, urgent or emergency situations, including initiation of emergency care.
* Review, prescribe and administer medication in line with national guidance and local formularies, using EMIS guidance and cost-saving prompts.
* Recognise safeguarding responsibilities for children, young people and adults and comply with statutory reporting requirements; ensure patient protection from abuse and uphold human rights.
* Work within own competence and scope of practice; report and manage clinical incidents.


Qualifications & Competencies

* Competencies across all four pillars of advanced practice in clinical practice.
* Ability to work autonomously in home and community settings with appropriate supervision as needed.
* Experience in leading prescribing and clinical governance in primary care settings.
