Main duties of the job

The way the NHS works is changing; care outside of traditional hospital settings is quickly becoming a much larger part of what the NHS does.

The Rapid response therapy team provides a same day, next day response to patient needs and supports hospital discharges within the community.

The aims of the service are to avoid hospital admissions where appropriate, support early hospital discharges and provide a short term therapy (up to 2 weeks) assessment and intervention to patients within their own homes.

Our service provides triage for community therapy services and is also part of the Falls Recovery service within our locality.
