An exciting opportunity has arisen for a Diabetes Specialist Nurse (DSN) to be at the forefront of driving up standards and quality in diabetes care in Swansea Bay University Health Board.To provide efficient and effective professional care in the specialised field of diabetes are, ensuring standards of patient care are consistently maintained in accordance with agreed operational policies.
Responsible for promoting and safeguarding the wellbeing and interests of all patients, staff and visitors to the service. To strive to improve working practice to increase efficacy and success rates. To work independently as a specialist nurse in their own right.
The post holder will be responsible for the day-to-day management of Diabetes patients in Singleton Hospital and will be required to work in line with the Health Board’s values and behaviour framework of caring for each other, working together and always improving.
The post holder is expected to work with autonomy to provide highly specialised professional clinical advice to colleagues to ensure high standards of direct care delivery.
The post holder will work collaboratively with the multi-disciplinary team that supports Diabetes patients in Singleton Hospital, including consultants, dietitians, admin support and wider Diabetes nursing and clinical teams across the Swansea Bay Health Board.
Main duties of the job
The post holder will provide specialist nursing care, advice, guidance and leadership deploying their specialist theoretical and practical
knowledge to;
• Assess, plan, implement and evaluate evidenced-based nursing care.
• Provide specialist advice and excellent clinical practice.
• Work autonomously and manage own workload as a lead specialist in their area.
• Provide specialist education and training to Staff.
• Work collaboratively and co-operatively with clinical colleagues.
• Demonstrate the Health Board’s values and ensure that others demonstrate the same values.
• Undertake Audit; CPD; teaching; research; administration, Clinics; ward-based work; telephone consultations etc.

3. Proficient level of knowledge in clinical speciality
