Main duties of the job
As a Senior Diabetes Nurse Specialist, you will:
1. Manage a caseload involving high-level assessment, advanced diabetes knowledge, diagnosis, initiating treatments, and coordinating care with multidisciplinary teams, including mental and social care.
2. Work closely with primary care, community services, and acute trusts to meet national diabetes care standards and promote wider changes in diabetes and nursing care.
3. Serve as an expert resource to the multidisciplinary team.
4. Drive change to support integrated care pathways across primary care, mental health, social care, and acute trust providers.
5. Enable access to appropriate services for people with diabetes.
6. Be an active member of the primary care team, developing caseloads and evolving practice based on population needs and national guidelines.
