This is your chance to be part of a vibrant team, dedicated to providing exceptional care to patients in the comfort of their own environments. In this role, your autonomy will shine as you assess and attend to the diverse health needs of our community, including delivering exceptional care to those facing end-of-life challenges and patients with long-term or complex conditions. Working under the guidance of the Nursing Team Leader, you'll leverage your clinical expertise to make vital decisions that directly impact patient outcomes.


Role and Responsibilities

* Provide hands-on patient care while mentoring and developing emerging nursing talent.
* Support professional oversight and ensure the safety and quality of care.
* Collaborate with the wider integrated neighbourhood team in an MDT approach to address diverse wellbeing needs.
* Hold the relevant CSP Qualification and have the required community nursing experience as set out in the job description.
* Deliver patient-centred care aligned with the organisation's values of pride, respect, empathy, consideration, compassion and dignity.
* Contribute to 7-day Community Nursing Service with variable shift patterns (8am–10pm).
* Possess a full driving licence and have access to a car.

The Community Nursing Service is a 7 day service with variable shift patterns available between 8am-10pm. Due to the nature of the role, it is essential that you possess a full driving license and have access to a car.
