Responsibilities
This is your chance to be part of a vibrant team, dedicated to providing exceptional care to patients in the comfort of their own environments. In this role, your autonomy will shine as you assess and attend to the diverse health needs of our community, including delivering exceptional care to those facing end-of-life challenges and patients with long-term or complex conditions. Working under the guidance of the Nursing Team Leader, you’ll leverage your clinical expertise to make vital decisions that directly impact patient outcomes. Your days will be dynamic and fulfilling, blending hands-on patient care with the gratifying task of nurturing emerging nursing talent, professional oversight and ensuring the safety and quality of that care. Working closely with members of our wider integrated neighbourhood team, you will be part of an MDT approach to addressing the diverse wellbeing needs of our population.
* Assess and attend to diverse health needs of the community, including end-of-life care and long-term/complex conditions.
* Work under the guidance of the Nursing Team Leader to make vital clinical decisions affecting patient outcomes.
* Provide hands-on patient care while mentoring and developing emerging nursing talent.
* Collaborate with the wider integrated neighbourhood team as part of an MDT approach.
Qualifications
You will hold the relevant CSP Qualification and have the required community nursing experience as set out in the job description. With patient-centred care as your main focus, you will demonstrate our values with pride, respect, empathy, consideration, compassion and dignity.
Location and Service Details
The Community Nursing Service is a 7 day service with variable shift patterns available between 8am-10pm. Due to the nature of the role, a full driving licence and access to a car are essential. The job is based at Woodsend Clinic, Urmston, Manchester, M41 8AD.
Qualifications and Experience
Essential
* First Level Registered nurse
* Community Specialist Practitioner (CSP) qualification
* Registered Non-Medical Prescriber
Desirable
* Masters level education
* Practice teacher training
* Other relevant course (e.g. care of the elderly/palliative care/ management)
Experience
* Evidence of continuous professional development
* Evidence of participation in clinical audit
* Implementation of audit recommendations
* Evidence of excellent communication, diplomacy and negotiation skills
* Evidence of leadership skills and involvement in change management
* Excellent written and verbal communication skills
* Teaching skills
