My client, a leading public sector organisation based in Belfast, is currently seeking a Full-Time Clinical Pharmacist.

Hours: Monday - Friday 9am to 5pm (Client will consider Part-Time)

Pay: £ per hour

Job Summary

The post holder will be responsible for providing Clinical Pharmacy Services to ensure the safe, clinically effective, and cost-effective use of medicines.

The post holder will be a member of the pharmacy team providing a clinical pharmacy service to the Belfast Health and Social Care Trust. To meet service needs and Trust objectives, the post holder may either:

1. Be based on a fixed ward/area as a clinical pharmacist, or
2. Gain rotational experience across medical, surgical, and other core clinical specialties, with rotations structured to offer clinical experience while balancing service needs.

Key Duties & Responsibilities

For each of the following, the post holder will:

Clinical and technical

* Provide clinical pharmacy services in various specialties, including wards, dispensaries, and outpatient clinics.
* With appropriate qualifications, act as an Independent Pharmacist Prescriber within an agreed area, within the scope of individual prescribing activities approved through the Trust Non-Medical Prescribing (NMP) process.
* Demonstrate and communicate specialist clinical knowledge.
* Assist in promoting safe, effective, and timely prescribing and administration of drugs.
* Support prescribing initiatives across the Belfast Trust.
* Obtain accurate medication histories on admission and perform medicine reconciliation.
* Provide comprehensive prescription monitoring, including identification and prevention of adverse drug reactions, drug interactions, therapeutic drug monitoring, and rationalisation of medicines, utilizing patient notes and blood results to ensure safety and efficacy.
* Manage Patients Own Drugs (PODs) where appropriate.
* Check discharge prescriptions at ward level and ensure timely discharge information for primary care professionals.
* Provide medication counselling and education to patients and carers.
* Offer medicine-related information and advice to healthcare professionals.
* Contribute to developing protocols, policies, patient group directions, and new drug requests.
* Advise on and support the secure storage of medicines at ward level, in compliance with legislation and Trust policies.
* Support risk management principles.
* Analyze, review, and report on drug usage and expenditure data.
* Identify new prescribing practices impacting drug expenditure (horizon scanning).
* Negotiate and influence practice to minimise drug expenditure and improve cost efficiency.
* Support development of clinical pharmacy services aligned with national standards.
* Participate in audits, monitoring, and research related to pharmacy services and medicines management.
* Provide statistical activity data regularly, including clinical interventions, to meet targets set with management.
* Participate in clinical audits, trials, and R&D activities.

Management and Leadership

o Review and guide the performance of subordinate staff annually.
o Participate in recruitment and selection panels.
o Organise own work according to objectives and deadlines.

Educational Responsibilities

o Act as a tutor or mentor for postgraduate staff.
o Support training of pharmacy staff, including junior and Foundation Year pharmacists, undergraduates, and technicians.
o Provide education to medical, nursing, and other healthcare staff.
o Maintain a CPD portfolio and pursue personal development.
o Participate in mandatory training and education programs.

Essential Criteria

o At least 18 months post-registration experience as a qualified hospital pharmacist and completion of a recognised Foundation Programme or equivalent.
o Membership of the Pharmaceutical Society of Northern Ireland or eligibility for membership.
o Ability to respond to emergency calls within 30 minutes, with access to transport, or suitable alternative arrangements if unable to drive.

Desirable Criteria

Additional criteria include effective communication, enthusiasm, self-motivation, research interest, and computer literacy. These will be assessed during application and interview.
