The post holder will be responsible for the assessment and treatment of patients with complex health and social care needs. Ensuring provision of a holistic nursing assessment and high quality, safe, evidence-based care to patients and carers with the aim of maintaining patients in their own homes.
Job Overview
The successful applicant will be an autonomous practitioner with advancing skills to care for acutely unwell adults. The post holder will work in this adaptable and evolving role, focusing on safe effective admission avoidance and supporting timely discharge from the acute and community hospitals, with a whole system approach.
Main Duties
* To be accountable to the Digital Health Ward manager when undertaking duties as a senior nurse.
* To work in an integrated way with all other agencies and other members of the multi-disciplinary team, across organisations.
* To oversee and take charge of the Virtual Ward caseload and delegate to other team members.
* To have an awareness of the need for the effective use of resources.
* To be a safe, effective and responsible lone worker.
Person Specification
Essential criteria: Registered Nurse (Adult), Evidence of recent relevant personal professional development, Post graduate Certificate in Long Term Condition or working towards, The ability to provide skilled, evidence-based nursing care to a wide range of patients, The ability to manage the caseload, Previous community experience.
* Leadership skills.
* Able to motivate a range of staff and organise self and others (delegating and prioritising workload/duties accordingly.
Desirable criteria: Teaching and assessing skills.
