Main duties of the job
As a Community Nurse, you will care for patients and their families, delivering high service standards, sharing best practices, and actively improving our ways of working. Job responsibilities
As a Band 5 Community Nurse, you will be an integral part of our team at Five Rivers Health Centre, working for the City Community Team.

You must be a registered Nurse with the NMC, able to work independently and within a multidisciplinary team. Strong communication, organizational skills, and flexibility are essential to deliver positive patient experiences.
You will need a full UK driving license and access to a car, as well as proficiency with IT systems and Microsoft Office.
