Pharmacy Technicians play animportant role in Primary Care, complementing clinical and communitypharmacists and other members of the practice multidisciplinary team.
The purpose of this role is tolead improvements to maximise safe, cost-effective best practice in prescribingto improve patient care.The role involves reviewing and recordingthe medication the patients should be taking, ensuring sufficient supplies,administering medicines, and counselling patients to support theirunderstanding on how to use their medicines safely.
Optimising and rationalisingmedication, switching medications to agreed and approved protocols, improvingrepeat prescribing processes, including promotion of repeat dispensing, onlineordering, minimising clinical risk and aiming to reduce waste in medicines.
The individual will support medication changes from secondary care or community specialists inline with the practice policies and actioningany medication changes that are required after reviewing hospital dischargeand clinic letters.
This is a part/full time vacancy - minimum 27 hours per week and the successful candidate will be based in practice.

Work with the ClinicalPharmacist, Community Pharmacists and Clinicians in accordance with agreedpractice prescribing standard operating procedures.
Process repeat medicationrequests, including the conversion of acute requests to repeat medicines,changing quantities of medicines.
Issuing prescriptions in linewith agreed clinical protocols and within your competency
Regular liaison with communityand acute hospital pharmacists.
Provide advice and guidance topatients regarding medication optimisation and lifestyle e.g. inhalertechniques.
To provide medication reviewservices to patients via clinics in the practice
To support the achievement ofthe practices prescribing Quality and Outcomes Framework targets.
Dealing with medication queriesincluding acute medication requests, patient medication and prescriptionqueries.
To update and maintain accuratepatient medication records on the practice clinical computer system, includingadvice given and action taken.
Liaise with secondary andprimary care colleagues to ensure correct medicines are continued following thetransfer of care.

* To work as part of the prescribing team providing support for the practice team (both clinical and administrative).
* To provide medication review services to patients via clinics in the practice.
* Help support the dispensary team by running DRUMS on the specified cohort of patients.
* Help support the practice with recalls.
* To support the achievement of the practices prescribing Quality and Outcomes Framework targets.
* To assist the clinical and management team with data analysis of prescribing behaviour in line with NICE guidance and local clinical commissioning directives.
* Improving repeat prescribing processes, including promotion of repeat dispensing, online ordering, minimising clinical risk and aiming to reduce waste in medicines.
* To support the Prescribing team by sharing appropriate medication guidance and establishing a method of communication to both administrative and clinical staff.
* Assist in the development of patient information leaflets.

* To support staff in the promotion of healthy lifestyles and support those with chronic diseases to develop good lifestyle changes to assist in the management of their conditions.

Qualifications
* Registered with the General Pharmaceutical Council as a Pharmacy Technician
* BTEC/NVQ Level 3 in Pharmacy services or equivalent Pharmacy Tech Qualification
* Educated to A-level/equivalent or higher or equivalent demonstrable experience
* GCSE English (C or above) and at least three others
