What You’ll Do: Conduct assessments and create tailored care plans to support adults with disabilities, ensuring their safety, independence, and well-being. Collaborate with healthcare providers, families, and community resources to deliver compassionate advocacy and interventions. Monitor and review cases, addressing complex needs such as mobility or mental health challenges with care and expertise. Maintain accurate records while adhering to Social Care Wales standards and safeguarding protocols. What We’re Looking For: A dedicated Social Worker with experience in adult social care, specialising in disabilities, and registered with Social Care Wales. Strong assessment and case management skills, with a person-centred approach and excellent communication abilities. Ability to work full-time (37 hours) and travel within Powys as needed. Candidates must reside within a reasonable commuting distance to Newtown, Powys. Essential Requirements: Eligibility to work in the UK.
