Join to apply for the Care Coordinator - CPN role at Greater Manchester West Mental Health NHS Foundation Trust. This is an exciting opportunity for a Qualified Mental Health Nurse, Social Worker or Occupational Therapist to join our specialist service for people experiencing a first episode of psychosis as a Care Co-ordinator.
Responsibilities
* Use sound clinical and risk assessment skills to plan and deliver interventions to high standards.
* Provide evidence-based interventions for young people experiencing a first episode of psychosis and promote recovery during the early stage of illness.
* Develop meaningful engagement with service users and their families and work in partnership with other services (primary care, education, social services, youth, and more).
* Ensure care is transferred thoughtfully and effectively at the end of involvement.
* Contribute to staff training to ensure knowledge and skills align with NICE concordant packages of care and Best Practice recommendations.
Qualifications
* Qualified Mental Health Nurse, Social Worker or Occupational Therapist.
* Genuine interest in providing evidence-based interventions for young people with a first episode of psychosis.
* Demonstrated ability to complete high-standard clinical assessments and interventions.
Requirements
* Experience in early intervention services or related mental health settings is desirable.
* Ability to work within an integrated service model for individuals aged 14 to 65 years.

* Pay enhancements: 30% for evenings (8pm onwards) and Saturdays; 60% for Sundays and Bank Holidays.
