As an Outreach Support Worker, you will support individuals in the community to take part in activities, events, and everyday experiences across the Calderdale area. Working alongside families and professionals, you will help create safe and empowering environments where people can build relationships, achieve personal goals, and live purposeful lives. Full training and ongoing support will be provided, including guidance on supporting individuals who may experience anxiety or emotional distress.
Hours
Variable contracted hours full & part time available.
Working days and evenings over 7 days on a rota basis to support an outreach/activity centre service.
Key Responsibilities
Deliver person-centred support in the community and at The Hub.
Support with communication, mobility, personal care, life skills, and activities.
Encourage participation in hobbies, community experiences, and personal development.
Build positive and trusting relationships with the people you support.
Maintain accurate records and work closely with families and professionals.
Promote safeguarding, dignity, inclusion, and emotional wellbeing.
Participate in training and ongoing professional development.
Skills & Experience
Calm, resilient, and able to work well in pressured situations.
Strong communication and teamwork skills.
Good level of physical fitness to support and participate in activities including swimming and trampolining.
Flexible to work evenings, weekends, and bank holidays.
A caring, person-led approach with patience and empathy.
Desirable
Experience supporting people with learning disabilities.
Full UK driving licence and willingness to drive minibuses.
Awareness of safeguarding, mental capacity, and health & safety principles.
Qualifications
Care Certificate or Level 2 Health & Social Care (desirable).
GCSE Grade C/4 or equivalent in English and Maths.
