West Lothian is investing in community assessment and rehabilitation through our Home First programme, including a new community single point of contact to improve patient pathways.
We seek experienced Band 3 / Band 4 Therapy Assistant Practitioners to join our multidisciplinary team, delivering person-centered assessments and therapy across community pathways, discharge planning, virtual community hospital wards, and step-down intermediate care facilities.
The successful candidate will join a stable, dynamic team committed to integrated working, focusing on patient outcomes and experience. This includes collaborating with inpatient teams to facilitate early supported discharge and community rehabilitation.
