Overview
This is an opportunity to gain experience within the Community Children's Nursing (CCN) Service based at Northampton General Hospital.
The post holder will manage a caseload of children/young people aged 0-18 years, assessing, planning and delivering evidence-based nursing care in the community setting across West Northants. The role involves working collaboratively with the child/young person, their parents/carers and the multi-professional team around the family (including interagency working). Enhanced communication skills are required to promote good working relationships to facilitate the best outcomes for the child/young person.
You will undertake lone working, risk assessments and you must adhere to local policies and procedures. You will deputise for the Lead Nurse in her absence, provide day-to-day management of the service, supervise junior team members and mentor students in training. Core service hours are 08.00-18.00, Monday to Friday, with contracted hours across early shifts 08.00-16.00 and late shifts 10.00-18.00 throughout the working week. The role requires flexibility to work across a 7-day service and may include unsocial hours and on-call for End of Life care.

Responsibilities
* Supporting Clinical Excellence: carry out a range of clinical skills in the community setting (e.g., enteral feeding, administration of intravenous therapies, injections, wound care; CVAD competencies).
* Set standards for assessment, planning, implementation and evaluation of patients' clinical care needs using agreed protocols; provide education and support for children and families in the community setting.
* Support patients, families and colleagues in highly distressing or emotional circumstances.
* Analyse and interpret presenting data and clinical presentation to meet complex needs, providing holistic care to the child and family; ensure timely referrals to other services when needed.
* Supporting Patient Excellence: ensure professional skilled nursing care in the community/home setting, adapted to individual needs; promote equality and value diversity; communicate with a range of professionals and non-professionals about sensitive information and difficult matters.
Organisational and Professional Responsibilities
* Act as a professional role model, provide clinical expertise to the team and other agencies; provide clinical supervision to staff and students; participate in recruitment and retention; in the Lead Nurse's absence manage sickness/absence in line with Trust policy.
* Maintain accurate contemporaneous documentation across systems; uphold the Nursing and Midwifery Council Code; represent the Directorate at external meetings and disseminate outcomes within the organisation.
Quality Assurance and Monitoring
* Promote, monitor and maintain best practice in health, safety and security; manage adverse events and complaints per Trust Policy; support high-quality, cost-effective specialist service through monitoring and audits.
Policy and Service Development
* Lead in creating and reviewing policies and guidelines to facilitate safe care for the child, family and community nurses; obtain feedback from service users and contribute to service development; appraise and apply recommendations to support the Lead Nurse in managing change.
