Job summary
The purpose of this role is to take ownership of the repeat prescribing process to ensure safe and efficient processes for patients and prescribers. Provide/support patient centred and holistic care as part of the Chronic Disease review process. Ensure safe prescribing occurs during transitions of care ( discharge from hospital). Be the first point of contact for medicine/prescribing queries from patients and other organisations. We are planning to interview for this role on Thursday 20th June from 12:00pm

Workalongside all GP practice staff as an integrated part of the team, undertakingnumerous medicines related activities.
Support reception staff to processrepeat prescription ordering. Review requests that come in, ensure that anyappropriate monitoring has been done or arrange this, contact patients tounderstand any issues that arise. Pass to prescriber for clinical review and issuingwith appropriate information attached.
Medicines reconciliation fromdischarge letters and out patient appointments to ensure timely and safechanges and address any concerns ahead of issuing by prescribers.
Help patients toget the best from their medicines by:-
o Dealing with ad-hoc patient and healthcare-related queriesregarding medications.
o Be involved with Annual Chronic Disease review process as part ofGP practice MDT by helping review and discuss any medication queries withpatients and support new medication initiation.
o Specific clinics to review medications to help understanding ofown medicines, support medication synchronisation, concordance.
Help ensurepractice is following appropriate prescribing guidelines via audit and reviewof safety alerts and new guidance.
Supportinitiatives for antimicrobial stewardship to reduce inappropriate antibioticprescribing.
Take a central role in running and enhancing practicescurrent processes on safe prescribing and monitoring of High Risk Drugs.
Take acentral role in the clinical aspects of shared care protocols and liaising withspecialist pharmacists for more complex patientsPromotion of electronic repeat dispensing and onlineordering
Work with localpharmacies to ensure good relationship for benefit of patients.
Support nurse andHCA with monitoring and regulations around storage of medications in surgery.
Teach studentsand GP trainees on how practice prescribing system works and their role withinit.
Support researchprojects that are being undertaken within the practice.
Workalongside all PCN Pharmacy staff as an integrated part of the team, undertakingnumerous medicines related activities.
Support themonitoring of medications in accordance with protocols and medication/patientrisk stratification tools
Help patients toget the best from their medicines by:-
o Liaising with PCN pharmacists with clinical queries
o Arranging SMRs both via audit process and ad-hoc for those thatneed
o Work with PCN Pharmacy Technicians to support audit work requiredfor PCN contract
o Participate in PCN pharmacymeetings to improve engagement between practice and PCN
Workalongside all ICB staff as an integrated part of the team, undertaking numerousmedicines related activities.
Carry out medicine switches in GP practices in line with practice and ICBagreed specific protocols reviewing patients records; amending records andinforming patients
Increase awareness of role of pharmacy technicianwithin the practice and to promote issues relevant to prescribing and medicinesoptimisation
Contribute tominimising clinical risks and aiming to reduce wasted medicines.
To undertake,participate and share the outcomes of clinical audits to facilitate improvementand changes in practice
Assist the practicein achieving national requirements, NICE implementation and utilisation ofmedicines optimisation initiatives.
General
To beresponsible for the organisation, planning and of own workload to meet setdeadlines
To utilise GPpractice clinical systems to conduct prescribing audits EMIS
Developrelationships with other pharmacy technicians, pharmacists and members of themulti-disciplinary team to support integration of the pharmacy team acrosshealth and social care including primary care, community pharmacy, secondarycare, and mental health
Handle function specific information, which may be sensitive, complex orconfidential and appropriately recording, transferring and/or coordinating suchinformation in accordance with the Data Protection Act; Caldicott Guidelinesand the Confidentiality Code of Conduct

8. BTEC/NVQ level 3 or equivalent in pharmaceutical sciences
