We are looking for a dynamic, motivated and enthusiastic person who has excellent communication, IT literacy and problem‑solving skills alongside a passion for patient‑centred care.
You will be part of an MDT team in which you will receive clinical and managerial support, and work alongside a wider MDT team that is aligned to social care professionals in reflection of the NHS 10‑year plan.

Responsibilities
* Deliver quality care to a designated caseload of people in the place that they call home, following a care plan devised by the community nursing staff.
* Provide concise information in a daily handover to ensure changing needs are met and risks are escalated.
* Support the MDT team and assist colleagues to cover service needs across the wider integrated team.
* Assist with clinical and managerial tasks as required under the guidance of the community nursing team.
* Attend a full induction programme and participate in ongoing training and competency development.
Qualifications & Requirements
* Excellent communication and interpersonal skills.
* IT literate and adept at problem solving.
* Passion for patient‐centred care.
* Access to a car is essential to carry out service duties.
* Flexible and able to cover a variety of shifts, including weekends, evenings and bank holidays.
Working Hours & Shift Pattern
The Community Nursing Service operates between 08.00 to 21.00 hours and the working pattern will include a variety of rotational shifts which will include weekend, evening and bank holiday working.
Location
South Bedfordshire (Dunstable and Leighton Buzzard). The posts are available to work alongside the Dunstable community nursing team delivering care to people living in Dunstable, Houghton Regis, Caddington, Toddington and surrounding villages; and the Leighton Buzzard Community nursing team, delivering care to people living in Leighton Buzzard, Linslade, Hockliffe and the surrounding villages.

2 × Full‑time positions (37.5 hours per week)
2 × Part‑time positions (1 × 18.75 hours per week and 1 × 15 hours per week)
