About The Role Support Worker - Support at Home

Location: Hull, working at base and in the community covering Hull and East Riding

Hours: per week

Salary: £, per annum

Contract: Permanent

Additional Requirement: Full UK driving licence held for a minimum of years and access to a vehicle with a willingness to take service users to and from appointments using your own vehicle, business milage paid

We are looking for a dedicated and enthusiastic Support Worker to join our service in Hull. If you have a caring, patient, and helpful nature you could make a tangible difference to someone’s life. You’ll work closely with hospital teams and partner organisations to help people return home safely after a hospital stay and maintain their independence.

Our support workers are a friendly face to someone who may be struggling to do things for themselves due to age, illness, or vulnerability. You'll allow people to enjoy healthier, more fulfilling lives. What You’ll Do

1. Respond to referrals and complete welfare calls and home visits.
2. Referrals to take the patient home from hospital to enable their discharge.
3. Provide assistance to a person returning from hospital or medical facility and those
identified by local health professionals as being at risk of a further decline in health or
without a support mechanism.
4. Attend hospital wards and discharge units to support patients.
5. Provide practical and emotional support to aid recovery and independence.
6. Assess service users’ needs, complete and follow support plans, identify safeguarding concerns, and accurately record case notes.
7. Signpost individuals to appropriate services and community resources.

This is a hands-on, community-based role where no two days are the same. You’ll need flexibility and confidence working in different locations across Hull.

To be a successful Support Worker, you’ll:

8. Have a full UK manual driving licence and access to a vehicle.
9. Have a good knowledge of services provided by the NHS and Social Care.
10. Contribute to the ongoing improvement of service quality and user outcomes.
11. Be professional. You can deal with queries in a diplomatic and confidential manner.
12. Be willing to work flexible hours to meet service needs.
