The Colne Practice, located in Rickmansworth, is seeking an experienced practice nurse to join their busy practice, preferably with a respiratory interest (asthma, COPD, spirometry). This position is offered on a part-time basis, for 2 days per week, working a Thursday and a Friday.
Main duties of the job
Working as a member of the multidisciplinary team, the successful individual will ensure nursing services are delivered effectively to the entitled patient population and implement processes for the management of patients with long‑term conditions.

The Colne Practice is a friendly GP practice, with a list size of approximately 10,500 patients. Our team comprises 7 GP partners, 2 practice nurses, 1 HCA, 2 clinical pharmacists and reception and admin teams. We have on site staff parking.
Job responsibilities
Main responsibilities will include:
* Be instrumental in the support of the Infection Prevention Control (IPC) lead and to work to the IPC requirements by providing audits, training, support and evidence of compliance to the practice manager.
* Diagnose and manage acute and chronic conditions.
* Provide wound care (ulcer/Doppler, etc.) to patients.
* Develop, implement and embed an effective care management programme for the frail and housebound patients registered at the practice.
* Assess the needs of patients ensuring the provision of healthcare is appropriate, incorporating evidence-based practice.
* Develop, implement and embed health promotion and wellbeing programmes.
* Implement and evaluate individual treatment plans for chronic disease patients that promote health and wellbeing.
* Identify, manage and support patients at risk of developing long-term conditions, preventing adverse effects to patients health.
Person Specification
Qualifications
* Current NMC registration with an active PIN.
All applicants should have the following qualifications, registrations, skills and experience:
* Experience of working within a primary care environment.
* Experience of working autonomously.
* Experience of infection control measures.
* Clinical knowledge including; ECGs, venepuncture, wound care, women's health.
* Mentor or teaching qualification.
* Experience of working as a practice or community nurse.
