Overview
This exciting opportunity is for a Band 6 Occupational Therapist to work within our Frailty Therapy team across Front Door Therapy Services, which covers the Frailty Assessment Unit and areas aligned with our Emergency Care department.
The team comprises Occupational Therapists, Physiotherapists, Therapy Assistant Practitioners and Discharge Managers who are vital in the assessment and planning of therapy needs and discharge arrangements for patients admitted through our Accident and Emergency department. We are part of the wider Frailty MDT with whom we have a close working relationship to ensure those living with frailty have holistic, comprehensive assessments and timely treatment interventions in the most appropriate environment.

Main duties
The team see patients admitted to the Frailty Assessment Unit which includes a Frailty SDEC area as an extension of our Accident and Emergency Department at the William Harvey Hospital. The post holder will provide comprehensive assessment and treatment for patients referred for Occupational therapy and work as an integral part of the multi-disciplinary team. They will contribute to delivering therapy and support comprehensive discharge planning, ensuring the patient\'s wishes are at the forefront of decision making.
* To manage daily a clinical caseload of patients, and be responsible for the assessment, formulation and treatments and to give specialised therapy advice to members of the MDT.
* To work closely with our community partners to ensure optimal transfer of care.
* To deputise for Team Leader on clinical and managerial issues within the team.
* To be actively involved with training within the Department in support of new graduate Occupational therapists, student therapists, therapy apprentices and support workers alongside other senior members of the team.
