The post holder will assist with the delivery of care management to the service population and undertake tasks and duties delegated by a GP/ Nurse or other suitably qualified clinical professional strictly in accordance with service practice guidelines and protocols. This role will support a variety of services across the organisation.
The post holder will at all times demonstrate and behave in a manner in accordance with the organisation's policies and protocols.
Key Responsibilities
1. New patient registration checks, NHS Health Checks, including completing any necessary paperwork and referrals within the practice
2. BP Monitoring
3. Urinalysis
4. Phlebotomy
5. Prepare and maintain environments and equipment before, during and after patient interventions
6. Notify a GP/Lead Nurse/ANP of any significant patient issues
7. Ensure full and accurate recording in the patients computerised records
8. Carry out administrative tasks for example, but not limited to, call and recall activities and stock takes as directed by senior members of the service
9. Provide lifestyle advice to patients making any necessary referrals within the practice
10. Weight monitoring
11. Ensure specimens are labelled and bagged ready for collection with the necessary paperwork completed
12. Simple woundcare dressing
13. Ensure clinical waste collection runs smoothly, reporting any problems to the nurse manager
14. Ensure sharps bins are ready for collection
15. Restocking / maintenance of equipment
16. Restocking of clinical areas and consulting rooms
Alongside these duties the post holder will need to report potential risks identified, be aware of statutory health procedures, understand and apply the principles of the cold chain, and work in accordance with the organisation's manual handling policy.

This role is a full time (Monday - Friday working days) HCSW role, with a total of 37.5 working hours per week. The wage is £13.83 per hour. The role will be primarily based at West Heath Surgery, B31 3HB.
