Overview
We have an exciting opportunity for an enthusiastic Band 5 Occupational Therapist to join our established Stroke Early Supported Discharge (ESD) Service based in the community. We work closely with acute stroke services in our area and our neurology colleagues also in the community to provide continuity of care across the stroke pathway. Our aim is to promote recovery and well-being, whilst empowering patients to self-manage and maximise their independence using health coaching techniques and meaningful activity. We support safe discharges home and facilitate rehabilitation therapy in patients' places of residence. Regular supervision will be provided to support your ongoing learning and developmental needs.
Responsibilities
* Manage your own caseload, whilst collaborating with the multidisciplinary team and other partner organisations.
* Utilise occupational therapy skills across domains of self-care, productivity and leisure.
* Conduct assessments and devise rehabilitation programmes aligned to patients' goals.
* Provide education, emotional support and carer support.
Qualifications & Experience
We are looking for a qualified occupational therapist who has real passion for working with stroke patients, wanting to make a difference to a patients rehabilitation and empowering them following their discharge from hospital.

Team
The immediate MDT team includes occupational therapists, physiotherapists, a speech and language therapist and therapy assistant practitioners. We are a small but passionate team, dedicated to provide high quality care and supporting our patients once they come out of hospital into their usual place of residence for continuing rehabilitation (Home / Care home).
